2. Submit the forms together with all the necessary attachments to:

3. We will review the application.
4. Once approved – we will send your approval by email.
5. Please give us a list of your needed items (limit 4):
   1. We will give you two (2) items: one (1) big item and one (1) small item (or group of small items – example, baby bottles).
   2. Please give four (4) choices: two (2) big and two (2) small items so you have a better chance of getting what you need.
   3. Once the item becomes available, we will contact you by email together with instructions where and when distribution will be.

      Please note: As we have no control on WHAT items are donated to us or WHEN they will be available, we can only give you what we have in hand.
      Recipients will be selected weekly.

6. Once we have your two (2) needed items it will be distributed to you.
